Job summary

Ennoble Care in New Jersey is seeking an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Monmouth County, NJ. You will apply medical techniques under physician supervision and care for a diverse set of patients with chronic diseases.

The role requires NJ NP certification, strong clinical skills, and the ability to work independently, with a focus on patient-centered care and collaboration with families and caregivers.

Qualifications

• Board-certified NP with NJ certification
• 1–2 years of primary care experience with geriatric patients
• Ability to work independently
• Strong clinical assessment and diagnostic skills
• Excellent communication with patients and families

Responsibilities

• Provide high quality care to primary and palliative patients in Monmouth County, NJ
• Assess patient's health by performing physical examinations; obtain and update medical histories and create care plans
• Communicate with office staff and clinical operations across internal platforms
• Develop impressions, diagnosis, and treatment plans
• Attend quarterly medical staff meetings
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications and interventions as needed
