Job summary

Ennoble Care is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in the Philadelphia region. You will practice under physician supervision and care for a diverse patient panel with chronic diseases.

Qualifications

• Board Certified NP or PA with PA state certification.
• Strong clinical skills to assess, diagnose and treat.
• Positive demeanor and strong people skills.
• Ability to problem solve and operate independently.
• 1–2 years of primary care with geriatric patients; hospice preferred.

Responsibilities

• Provide high quality primary and palliative care to homebound patients in and around Philadelphia, PA.
• Perform exams, take histories, and develop care plans.
• Communicate with office staff and clinical operations across platforms.
• Deliver patient centered care and build trust with patients, families, and caregivers.
• Order diagnostic tests and prescribe medications and interventions as needed.
• Develop impressions, diagnoses, and treatment plans; collaborate with physicians.
