Job summary

UnitedHealth Group is hiring an in-home Advanced Practice Clinician (NP/PA) to conduct annual Medicare Advantage assessments. You will work in a house-call setting, coordinating care with primary care providers and other health professionals. Travel up to 85% is required to visit members in their homes.

The role emphasizes education, gap closure, and collaboration to improve health outcomes, with flexible scheduling and a sign-on bonus for external candidates.

Qualifications

• Active NP or PA license or ability to obtain by start date.
• ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or NCCPA for PAs.
• Ability to complete physical requirements (moving 30-lb bag, stairs, varied dwelling conditions).

Responsibilities

• Conduct in-home assessments including medical history review, vitals, and exam.
• Perform point-of-care testing as applicable.
• Identify diagnoses for care management and therapeutic planning.
• Document findings to inform the member's PCP of care gaps.
• Identify urgent or emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Travel up to 85%.
