Job summary

UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage and other plans. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes and access to care at home.

You will work in an interdisciplinary team with a focus on preventive care, patient education, and care coordination.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (NPs).
• Current NCCPA certification and state licensure (PAs).
• Active, unrestricted licensure in the practicing state; no disciplinary actions.
• Active and unrestricted driver''s license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments (sit/stand/kneel).
• If state allows independent NP/PA practice, obtain licensing board approval within required timelines.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members'' PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
