UnitedHealth Group is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

The position offers flexible scheduling, no on-call/weekends/holidays, and requires licensure in the practice state, with travel to patients' homes and proficiency with EMR systems.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriartric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure for PAs.
• Active and unrestricted driver’s license with reliable transportation for home visits.
• Ability to obtain licensure in the practice state by start date.

Responsabilités

• Conduct comprehensive in-home health assessments for Medicare Advantage and other members.
• Review history, medications, vital signs, and perform physical exams.
• Document diagnoses and communicate findings to PCPs to close gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
