UnitedHealth Group’s Optum HouseCalls team is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role focuses on education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible scheduling between 8am–8pm, 7 days a week, with no on-call, weekends or holidays.

Qualificações

• Active nursing or physician assistant license in good standing
• Certification requirements: ANCC or AANP for NPs or NCCPA for PAs
• State licensure without disciplinary actions
• Driver's license and reliable transportation for home visits
• Ability to work independently in varied home environments
• Fluency in languages other than English is a plus

Responsabilidades

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings to primary care providers to close care gaps
• Educate members on disease processes, medications and adherence
• Identify urgent situations and intervene appropriately
• Address social determinants of health and arrange referrals when needed
