Job summary

Optum Home & Community Care, part of UnitedHealth Group, seeks a HouseCalls Advanced Practice Clinician (NP or PA) for in-home Medicare assessments in Erie/Niagara. Flexible 8am-8pm scheduling, seven-day week coverage with no on-call, weekends or holidays.

Competitive compensation includes base pay with bonus; potential $40,000 student loan repayment or $30,000 sign-on bonus; language incentives for multilingual providers; comprehensive benefits are offered.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Primary Care.
• Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state w/o disciplinary actions.
• Driver’s license with reliable transportation for home visits.

Responsibilities

• Conduct annual in-home health assessments for Medicare Advantage and other plan members.
• Perform medical history review, vitals, medications reconciliation and exam.
• Educate members on disease processes, medications, and adherence.
• Identify urgent situations and coordinate with PCPs to close gaps in care.
• Document findings and support care management and planning.
