Job summary

Optum HouseCalls is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes, with no prescribing involved.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (NPs).
• Current NCCPA certification and state licensure (PAs).
• Active, unrestricted licensure in the practicing state, without disciplinary actions.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history review, medication reconciliation, vital signs, and physical examination.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps and coordinate with primary care teams.

Education

ANCC or AANP Family/Adult/Geriatric Primary Care Certification

NCCPA Certification

State licensure (active, unrestricted)
