UnitedHealth Group’s Optum Home & Community Care, part of the Optum family, is creating something new in health care. As a HouseCalls Advanced Practice Clinician (NP or PA), you will perform in-home health assessments for Medicare Advantage members, educating patients and coordinating care with PCPs.

This field-based role offers flexible scheduling, no on-call, and a robust benefits package including a market-range compensation and growth opportunities within the Optum HouseCalls team.

Qualifications

• Active NP/PA certification with current licensure.
• Experience in home health or geriatrics preferred.
• Ability to obtain licensure in the practicing state by start date.

Responsabilités

• Conduct in-home assessments including history, medications, vitals, and physical exam.
• Document diagnoses for care management and treatment planning.
• Coordinate with PCPs to close care gaps.
• Identify urgent or emergent situations and intervene appropriately.
• Educate patients on diseases and medications; address social determinants of health.
