Qualifications

• Active state license as NP/PA or license to practice
• MSN required for Nurse Practitioners; PA may not require a master’s
• Maintains current CPR certification
• Active DEA license
• 5+ years nursing healthcare experience; 2+ years NP/PA experience preferred
• HIPAA/OSHA knowledge and compliance

Responsibilities

• Provide comprehensive primary care in home settings
• Conduct clinical assessments and develop personalized care plans
• Collaborate with interdisciplinary teams for seamless care transitions
• Manage chronic conditions and preventive care
• Order and interpret labs and diagnostics as needed
• Maintain accurate patient records and referrals
