Qualifications

• Develops personalized care plans and coordinates with interdisciplinary teams
• Manages chronic conditions and acute visits with focus on prevention and wellness
• Maintains up-to-date medical records and documentation
• Collaboration with caregivers and other healthcare professionals to ensure seamless transitions
