Job summary

Abode Care Partners is seeking a Nurse Practitioner to join our Mechanicsville, VA team providing comprehensive primary care to seniors in home and community settings.

We offer a $10,000 sign-on bonus, performance incentives, and a comprehensive benefits package. You will conduct clinical assessments, develop care plans, manage chronic conditions, and collaborate with an interdisciplinary team to improve patient outcomes in skilled nursing and assisted living environments.

Qualifications

MSN required for NP; national NP/PA certification preferred.
Active RN license in good standing; NP/PA licensure as applicable.
Current CPR and state DEA licensure; HIPAA/OSHA knowledge.

Responsibilities

Performs clinical assessments and develops personalized care plans.
Manages chronic and acute conditions and coordinates with interdisciplinary team.
Orders and interprets laboratory and diagnostic tests as needed.
Maintains up-to-date medical records and provides referrals when necessary.
