Job summary

Abode Care Partners is seeking a compassionate Nurse Practitioner to provide comprehensive primary care in patients' homes around Richmond, VA. You will conduct clinical assessments, develop personalized care plans, and collaborate with interdisciplinary teams to ensure seamless transitions and optimal outcomes.

Qualifications

• Active state license as NP or PA; CPR current.
• NP requires MSN; PA does not.
• DEA licensure in good standing.
• RN license in state employed if NP.
• Strong communication and time management skills.

Responsibilities

• Performs evidence-based medical care and develops care plans.
• Examines chronic and acute illnesses, minor injuries, and injuries.
• Orders/interprets labs and diagnostics.
• Collaborates with interdisciplinary teams and facilitates care transitions.
• Maintains up-to-date medical records.
