Job summary

Abode Care Partners in Richmond, VA seeks a Nurse Practitioner to join our home-based primary care team. You will conduct clinical assessments, develop care plans for seniors in skilled nursing, assisted living, and private homes, and collaborate with an interdisciplinary team to improve outcomes.

Qualifications

• Active state license as NP or PA.
• MSN for NP; PA does not require MSN.
• Current CPR certification.
• DEA licensure active.
• 5+ years nursing healthcare experience; 2+ years NP/PA preferred.
• Valid driver’s license.

Responsibilities

• Plans, directs, and evaluates patient medical needs.
• Treats chronic and urgent illnesses; minor injuries.
• Orders and interprets laboratory and diagnostic tests.
• Collaborates with caregivers and other health professionals.
• Provides referrals as necessary.
• Maintains up-to-date medical records.
