Job summary

Your Health in Myrtle Beach seeks a Transitional Care Professional to coordinate patient transitions from hospitals and facilities to home. You will perform home and facility visits, arrange services, and work with the care team to ensure smooth transitions and reduce readmissions.

The role requires reliable transportation for daily travel, full-time hours, and 8-hour shifts Monday-Friday (8am-5pm).

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse with good standing.
• Minimum of three years clinical experience preferred.
• Strong written and verbal communication skills and time management.

Responsibilities

• Visit referrals and established patients within assigned facilities.
• Establish trust with patients to aid informed decisions about their health.
• Coordinate care by arranging home health, outpatient rehab, and follow-up with providers and staff.
• Schedule all essential visits including primary care, specialists, pharmacists and social workers.
• Facilitate effective communication between healthcare professionals involved in care.
