Job summary

Your Health in Marietta is seeking a Transitional Care Professional to coordinate patient transitions from hospitals and facilities back home. This full-time role involves home and facility visits in the Marietta area, with daily travel and an 8-hour shift from 8am to 5pm, Monday through Friday.

A successful candidate will have clinical experience, licensure in nursing or related field, and strong communication skills to support patients, families, and care teams.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse in good standing.
• Minimum three years of clinical experience preferred.
• Valid driver's license and reliable vehicle required.
• Strong communication and organizational abilities.

Responsibilities

• Visit referrals and established patients in assigned facilities and homes.
• Establish trust with patients to support informed health decisions.
• Coordinate care with the care team and providers to ensure seamless transitions home.
• Arrange services such as home health, rehab, and follow-up care.
• Maintain timely scheduling of essential visits with providers and staff.
• Communicate effectively among healthcare professionals involved in care.
• Deliver compassionate, patient-centered service and manage patient concerns promptly.
• Perform general administrative tasks and support staff as needed.
• Utilize company software to enhance patient care and staff efficiency.
• Travel daily between offices and facilities as required.
