Job summary

Your Health Organization in Myrtle Beach, SC is seeking a Transitional Care Professional to coordinate patient care from hospitals and facilities back home. This full-time position involves travel to homes and facilities and requires reliable transportation.

Shifts are 8 hours, Monday through Friday, 8am–5pm. Responsibilities include coordinating home health, rehab, and follow-up care, scheduling visits, and maintaining clear communication with care teams.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred with licenses in good standing.
• Prior public health or home health experience preferred.
• At least 3 years of clinical experience preferred.
• Ability to supervise and direct professional and administrative staff.
• Effective reading, writing, and communication skills.
• Strong organizational, time-management, and multitasking abilities.
• Basic computer skills and familiarity with software systems.

Responsibilities

• Visit referrals and established patients within assigned facilities.
• Build trust with patients to aid informed health decisions during facility stays.
• Provide guidance based on care team recommendations.
• Coordinate seamless home transitions with home health, rehab, and follow-up care.
• Schedule all essential visits including primary care, virtual visits, specialists, and pharmacists.
• Facilitate communication among healthcare professionals involved in care.
• Ensure adherence to the plan of care by patients and families.
• Interact with patients via phone, email, portal, or in person to gather information.
