Amerita, a BrightSpring Health Services affiliate, seeks a Pharmacist to join our home infusion pharmacy team in the Charlotte, NC area. The pharmacist will evaluate drug therapy, ensure compliance with regulations, and support patient care in the home setting.

Responsibilities

• Evaluates and monitors drug therapy for home care patients in collaboration with physicians, nurses and other healthcare professionals.
• Adheres to organization policies, procedures, and state/federal laws governing pharmacy practice.
• Coordinates and facilitates new referrals and admissions with nurses, customer service and distribution.
• Reviews and verifies all new patient orders and documents on the order form.
• Assists physicians and healthcare professionals with planning and implementing patient care at home and monitoring therapy objectives.
