Job summary

Amerita, Inc. in Florida is seeking a Pharmacist to join our Pharmacy team within the home infusion space.

The Pharmacist will report to the Pharmacy Manager and work in our Florida branch, coordinating with physicians, nurses and distribution to deliver patient-specific therapy. Responsibilities include evaluating drug therapy, ensuring USP standards, supervising technicians, and preparing sterile admixtures.

Qualifications

• Min 2 years acute-care or 1 year home infusion experience.
• Experience with compounding infused drugs.
• Excellent verbal and written communication skills.
• Travel up to 25% - Minimal travel may be required.

Responsibilities

• Evaluates and monitor drug therapy for home care patients with physicians, nurses and other healthcare professionals.
• Adheres to policies, procedures and all applicable state/federal laws.
• Coordinates referrals and admissions with nurses, customer service and distribution.
• Reviews and verifies new patient orders and documents them.
• Supports physicians and other healthcare professionals with home care planning and monitoring.
• Conducts patient, drug and disease information assessments to identify drug-related problems.
• Monitors drug therapy outcomes using labs and clinical data.
• Provides drug information to patients, families, and clinicians.
• Maintains documentation of clinical activities and communication in patient records.
• Prepares medications and supplies for dispensing; ensures aseptic techniques.
• Maintains USP 797 and USP 800 compliance in clean rooms and related areas.
• Oversees Pharmacy technician functions as required.
