Job summary

Druid City Infusion, part of the Vital Care Infusion Services network, is hiring a Home Infusion Pharmacist in Metairie, LA. Qualifications

• PharmD or Bachelor of Science in Pharmacy from an accredited institution.
• Current and unrestricted state pharmacist license in good standing.
• Current immunizations certification (if required by state law).
• BLS or CPR certification – not required.

Responsibilities

• Perform comprehensive medication reviews and patient consultations to optimize therapy.
• Collaborate with physicians and healthcare teams to develop therapy plans.
• Educate patients and caregivers on medication administration, storage, and safety.
• Monitor patient progress and adjust treatment in partnership with the team.
