Job summary

Druid City Infusion, part of the Vital Care Infusion Services network, is hiring a Home Infusion Pharmacist to oversee preparation, administration, and oversight of home infusion therapies. You will collaborate with patients, clinicians, and infusion nurses to deliver safe, high-quality pharmaceutical care in home and infusion center settings.

Requirements include a PharmD or B.S. in Pharmacy, an active state license, and immunizations certification; two years of pharmacist experience; and

Qualifications

• Pharmacology knowledge and infusion therapy expertise required.
• Licensed pharmacist with immunizations certification preferred.
• Experience with pharmacy management software and patient education expected.

Responsibilities

• Oversee preparation, dispensing, and delivery of home infusion medications.
• Educate patients and caregivers on therapy administration and safety.
• Collaborate with physicians, nurses, and healthcare teams on therapy plans.
• Monitor patient progress, document therapy adjustments and outcomes.
• Ensure compliance with federal/state laws and infection-control protocols.
• Manage inventory, vendor coordination, and safe disposal of medications.
