Job summary

Druid City Infusion, part of the Vital Care Infusion Services network, seeks a Pharmacist to oversee home infusion therapy programs in Tupelo, MS. Responsibilities include reviewing medications, educating patients, supervising sterile compounding, and coordinating with physicians to optimize therapy.

Applicants must have a PharmD or BS in Pharmacy, a current state license, and BLS/CPR certification as applicable. Experience with CareTend software is preferred.

Qualifications

• PharmD or Bachelor of Science in Pharmacy from an accredited institution.
• Current and unrestricted state pharmacist license in good standing
• Current immunizations certification (if required by state law)
• Basic Life Support (BLS) or CPR certification – not required

Responsibilities

• Perform comprehensive medication reviews and offer patient consultations to optimize drug therapy management.
• Assess patients’ clinical condition by reviewing lab results and vital signs to determine suitability of home infusion therapy.
• Collaborate with physicians and healthcare providers to develop individualized therapy plans.
• Educate patients and caregivers on medication administration, storage, side effects, and potential complications.
• Monitor and evaluate patient progress, adjusting treatment plans with the healthcare team.
• Supervise preparation, dispensing, and delivery of home infusion medications adhering to standards.
• Compound and label sterile products following safety protocols.
• Track patient outcomes and document therapy changes in records.
