Job summary

Druid City Infusion, part of the Vital Care Infusion Services network, is seeking a Home Infusion Pharmacist to ensure safe preparation, administration, and oversight of home infusion therapy. You will collaborate with patients, physicians, and infusion nurses to deliver high-quality pharmaceutical care in home and infusion center settings.

Requirements include a PharmD (or BS) from an accredited school, an active state license, and at least 2 years of pharmacist experience.

Qualifications

PharmD or BS in Pharmacy from an accredited institution.
Current, unrestricted state pharmacist license in good standing.
Current immunizations certification (if required by state law).
BLS or CPR certification - not required.

Responsibilities

Perform comprehensive medication reviews and patient consultations to optimize drug therapy management.
Supervise the preparation, dispensing, and delivery of home infusion medications, ensuring adherence to safety and regulatory standards.
Educate patients and caregivers on correct medication administration, storage, potential side effects, and complications.
Collaborate with physicians and healthcare providers to develop individualized therapy plans.
