Job summary

Druid City Infusion, part of the Vital Care Infusion Services network, seeks a Home Infusion Pharmacist to ensure safe preparation, administration, and oversight of home therapies. You’ll collaborate with patients, nurses, and physicians to deliver high-quality pharmaceutical care in home and infusion-center settings.

Requirements include a PharmD or BSc in Pharmacy, active license, and 2+ years of pharmacist experience; sterile compounding experience is preferred. Equal opportunity employer.

Qualifications

• PharmD or Bachelor of Science in Pharmacy from an accredited school.
• Current and unrestricted state pharmacist license in good standing.
• Current immunizations certification may be required by state law.
• Basic Life Support (BLS) or CPR certification - not required.

Responsibilities

• Perform comprehensive medication reviews and offer patient consultations to optimize drug therapy.
• Educate patients and caregivers on correct medication administration, storage, and potential side effects.
• Supervise the preparation, dispensing, and delivery of home infusion medications with safety protocols.
• Collaborate with physicians to develop individualized therapy plans for patients.
• Track patient outcomes and document therapy adjustments in records.
