Job summary

Druid City Infusion, part of the Vital Care Infusion Services network, is seeking a Home Infusion Pharmacist to ensure safe preparation, administration, and oversight of home infusion therapies.

You will collaborate with patients, physicians, and infusion nursing teams to educate on medications, monitor outcomes, and supervise sterile compounding and delivery of treatments such as antibiotics and nutrition support.

Qualifications

• Pharmacist must hold a PharmD or BS in Pharmacy from an accredited institution.
• Current, unrestricted state pharmacist license in good standing is required.
• Immunizations certification may be required by state law.
• BLS/CPR certification is not required.

Responsibilities

• Provide patient care and consultation to optimize drug therapy.
• Oversee preparation, dispensing, and delivery of home infusion medications.
• Monitor patient progress and document therapy outcomes in patient records.
• Collaborate with home health nurses, physicians, and other providers to coordinate care.
• Ensure compliance with federal/state laws and safety standards for home infusion.
• Manage inventory and coordinate with vendors for medications and supplies.
• Educate patients and caregivers on treatment administration and safety.
