Option Care Health, Inc. is seeking an Infusion Nurse Educator to deliver clinical education and training for patients transitioning to our home infusion care model. This RN role includes providing direct patient care in home or infusion suites, and partnering with the sales team for successful transitions.

Requires an active RN license, CPR certification, and at least 2 years of nursing experience. Some local travel to care facilities and patient homes is expected.

Qualifikationen

• Active and unrestricted RN license required.
• Minimum of 2 years nursing experience.
• Current CPR certification required.
• High School diploma or GED required.

Aufgaben

• Initiates nursing plan of care treatments and educates patients and families/caregivers.
• Assesses patient needs at each skilled visit and updates plans accordingly.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Communicates with clinical team, providers, patients, and families to ensure continuity of care.
• Coordinates discharge planning and prepares discharge summaries with instructions.
• Maintains compliance with policies, procedures, and regulatory standards.
• Uses computer systems to document care and support billing processes.
