Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or alternate infusion suite. You will implement plans, educate patients and families, and ensure safety under physician orders.

Travel to patient locations is required, with supervision by a nurse manager as appropriate. The role requires an active RN license, CPR certification, and at least 1 year of nursing experience, plus the ability to operate a vehicle for business

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification.
• Must have a valid driver’s license and access to a vehicle for business travel.

Responsibilities

• Initiates and implements intermediate nursing plan of care treatments.
• Coordinates discharge planning and prepares discharge summaries.
• Educates patients and families about care plans and therapies.
• Documents patient progress and communicates with the clinical team in real time.
• Performs nursing procedures and administers medications per physician orders.
• Ensures compliance with HIPAA and agency policies.
