Qualifications

• Licensed RN required to perform infusion care in home or infusion sites.

Responsibilities

• Initiates, develops, and implements basic nursing plan of care treatments; evaluates patient progress; provides direct patient care; educates patients and families; adjusts plan per physician orders.
• Assesses patient needs and status at visits; re-evaluates and clarifies physician orders; informs physician of changes.
• Provides patient- and family-centered teaching to achieve independence with prescribed therapy.
• Communicates with clinical team, physicians, patients, and families; documents care in real-time for billing and compliance.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
• Ensures compliance with agency policies, procedures, regulations, and accreditation standards.
• Uses computers and other tools to validate orders, document care, and follow visit schedules.
• Responds to patient requests; initiates emergency procedures as needed.
• Maintains professional conduct and aligns with Naven Health’s mission and policies.
• Maintains confidentiality per HIPAA.
• Educates patients about Naven Health services and plan of care.
• Completes required trainings and continuing education to maintain licensure and competency.
