Job summary

Option Care Health in Illinois is seeking a Clinical Transition Specialist, an experienced sales professional who educates discharge patients, families and facility staff about home infusion services and products, and ensures proper placement within the home health setting.

You will build relationships with referral sources, collaborate with intake and account teams, and drive growth by expanding referrals while delivering expert patient education and a seamless transition from hospital to home

Qualifications

• Experience establishing relationships at all levels of the organization.
• Experience applying knowledge of ACHC/URAC standards.
• Experience providing customer service to internal and external customers.
• Proficient with MS Office (Word/Excel/PowerPoint).
• Must have reliable transportation to travel to care facilities and hospitals.
• Ability to plan, organize and make presentations.

Responsibilities

• Grow relationships with referral sources to increase sales and patient starts.
• Interact with area providers to sell services that support discharged patients.
• Educate patients, families and facility staff about services and products.
• Provide hands-on in-person education to patients and caregivers for timely discharge.
• Communicate with agency staff and medical teams to implement treatment plans.
• Collect and review pre-admission information and obtain signoffs.
