Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients and discharge planning staff about home infusion services.

The role focuses on placing patients in the appropriate home or alternate site care, collaborating with intake, case managers, and discharge planners. As an experienced RN or Pharmacist, you will build relationships with referral sources and Account Managers to grow referrals and meet sales goals, while maintaining patient care quality and treatment

Qualifications

• Licensed RN or Pharmacist in the state of practice with healthcare experience.
• Experience establishing relationships with stakeholders and vendors.
• Experience delivering customer service to internal and external customers.
• Proficient in Microsoft Office applications (Word/Excel/PowerPoint).
• Ability to identify operational issues and drive process improvements.
• Valid driver’s license and reliable transportation.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Educates patients and facility staff about in-home infusion services and products.
• Coordinate with internal intake and external partners for smooth patient transitions.
• Provide in-person patient and caregiver education for timely discharge and therapy independence.
• Communicate with medical teams and families throughout the discharge process.
• Assist with pre-admission information collection and signoffs.
