Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients and care teams about home infusion services, and to build referrals by partnering with discharge planners and hospital staff.

The role focuses on placing patients in the appropriate home or alternate site setting to support timely discharge and independence in therapy. The position requires hot-line sales capability, collaboration with Account Managers and Regional Sales Directors, and ongoing education of

Qualifications

• Licensed RN or licensed Pharmacist in state of practice with healthcare experience.
• Experience establishing relationships at multiple levels of organizations and with vendors.
• Strong communication, presentation and customer service skills.
• Basic PC skills including internet, email and Microsoft Office suite.

Responsibilities

• Maintain and grow relationships with referral sources to increase sales and patient starts.
• Assess patients selected by hospital to ensure suitability for home/alternate site care.
• Educate patients, families and facility staff about services and products.
• Provide in-person education to patients and caregivers to support timely discharge.
• Coordinate with internal and external partners to implement effective treatment plans.
• Collaborate with Account Manager and Regional Sales Director to develop area growth plans.
• Communicate opportunities and progress to Account Manager and marketing staff.
• Review medical records for pertinent history and insurance information.
• Initiate care transition coordination to ensure seamless patient transitions to home/ATS.
