The role focuses on building relationships with referrals, collaborating with hospital staff, and guiding patients to the appropriate home or alternate site care. You will coordinate care transitions with internal teams and external partners, ensuring accurate information transfer, compliant processes, and timely therapy starts while supporting

Qualifications

• Licensed RN or Pharmacist with 2+ years in healthcare.
• Strong relationship-building with diverse stakeholders.
• Experience aligning with ACHC/URAC standards.
• Proficient in Word/Excel/PowerPoint and internet/email.
• Willingness to travel to facilities and hospitals.

Responsibilities
