Job summary

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient and family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with patient/family, attending physician and IDG members daily to provide care.
• Instructs and supervises the patient/family in self-care where appropriate.
