Job summary

Chapters Health is seeking a Registered Nurse, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative, and supportive care to keep patients in their home environment if possible.

The role requires an active RN license in the state of employment, at least 1 year of hospice or hospital experience, and strong communication skills for interactions with patients, families, and IDT members.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• BLS for healthcare professionals from American Heart Association or Red Cross required.

Responsibilities

• Provide and manage direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinate the Plan of Care reflecting patient’s evolving needs.
• Educate patients and families about disease, palliation, and safety practices.
• Perform home visits and assess safety, medication, nutrition, and DME compliance.
