Job summary

Chapters Health System in Florida seeks an experienced RN Case Manager to coordinate care for patients at home, partnering with the IDT to keep individuals safely in their homes. You will assess needs, develop the Plan of Care, and provide palliative support, with an emphasis on compassionate communication.

This role requires an active RN license in Florida, at least one year of nursing experience (hospice/hospital preferred), and valid driver’s license for home visits.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital experience preferred.
• EMR/EHR system experience.
• Mobile Driver with valid license and auto insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate in English (verbal and written).
• Ability to visit participants in their homes.
• Ability to meet essential functions with/without accommodation.
• Active BLS for healthcare professionals.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patient, family and caregivers about disease process, prevention, and palliative care.
• Reports changes in patient condition to IDT or health professionals.
• Presents reports to IDT and collaborates with disciplines.
• Supervises care by CHWs and HHAs as requested.
• May be required to work during emergencies to ensure continuity of services.
