Job summary

21st Century Home Health Services is seeking a Speech Therapist/Language Pathologist to provide in-home skilled services across Sacramento County. You will evaluate and treat speech, language, cognitive and swallowing disorders as part of an interdisciplinary team, guided by a Case Manager.

Requirements include CA licensure, BLS/CPR, and a CA Driver's License. The role offers flexible scheduling, autonomy, mileage reimbursement, and competitive compensation with benefits.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of development and human behavior as it relates to illness/injury; cultural health care influences.
• Knowledge of state/federal homecare regulatory guidelines preferred.
• Experience in home health care highly preferred.
• Experience with high acuity patients a plus.
• Experience with interdisciplinary team highly desired.
• Case Management fundamentals highly desired.

Responsibilities

• Provide skilled speech-language pathology services in patients' homes in partnership with the Case Manager.
• Evaluate and treat speech, language, cognitive and fluency disorders.
• Serve as educator, patient advocate and member of the interdisciplinary care team.
