21st Century Home Health Services in Northern California is seeking a Speech Therapist/Language Pathologist (ST) for a fully in-person home health role in Solano County. You will provide skilled ST services in patients' homes, collaborating with the Case Manager and interdisciplinary team to evaluate and treat speech, language, cognitive, and swallowing disorders.

Requirements include a CA Speech Therapist License and BLS/CPR certification.

Qualifications

• Active CA Speech Therapist license and BLS/CPR certification required.
• Experience with in-home speech therapy and interdisciplinary teams preferred.
• Knowledge of culture and health care in home settings.

Responsabilités

• Provide skilled speech therapy services in patients' homes in Solano County.
• Collaborate with Case Manager and interdisciplinary team to plan care.
• Evaluate, treat, and educate patients on speech, language, cognition, and swallowing.
• Serve as patient advocate and educator within the care team.
