Job summary

CenterWell Home Health in the United States is seeking a Speech Language Pathologist to join our home health team. You will evaluate and treat patients in the home or facility, participate in Plan of Treatment, and use your clinical skills to improve speech, language, voice, and swallowing.

This full-time role requires ASHA CCC-SLP, at least six months of experience, a current license, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology (ASHA) or Audiology
• Minimum six months experience as a speech therapist / SLP
• Current and unrestricted license
• Current CPR certification
• Valid driver’s license, auto insurance and reliable transportation

Responsibilities

• Evaluate, direct and provide speech/language pathology services in the home or facility
• Participate in the development and periodic review of Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and applications of therapeutic treatments
• Observe, record and report changes in patient condition to supervisor/physician
• Provide instruction on alternative communication systems when appropriate
• Counsel and instruct patients, families and healthcare staff
• Maintain and submit documentation and reports as required
• Participate in care coordination and discharge planning
• Maintain confidentiality and professional conduct
