Job summary

CenterWell Home Health, a Humana company, seeks a licensed Speech-Language Pathologist to deliver in-home therapy focused on rehabilitation and functional communication. Qualifications

• Meet the education and experience requirements for CCC-SLP.
• Minimum six months experience as a speech therapist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license and auto insurance.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient's condition and response to treatment.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation and progress reports as required by policy.
• Participate in care coordination and discharge planning.
