Job summary

CenterWell Home Health is looking for a Speech Language Pathologist to deliver home-based therapy, assess patients, and contribute to care plans. You’ll work under the Clinical Manager, supporting rehabilitation and functional improvement in the patient’s home environment.

The role requires ASHA certification or state licensure, a minimum of six months’ experience, a current license, CPR, and a valid driver’s license with insurance. Home health experience preferred.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology (ASHA) or Audiology per ASHA.
• Minimum six months of experience as a speech-language pathologist.
• Current and unrestricted professional license.
• CPR certification current and valid.
• Valid driver’s license with auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; document changes.
• Administer and interpret diagnostic tests and therapy applications; conduct screening.
• Maintain and submit documentation per company policy; discharge planning and care coordination.
