Job summary

Humana Inc. through CenterWell Home Health seeks a Speech-Language Pathologist to deliver home-based therapy, focusing on rehabilitation and functional independence for patients in their homes.

You will work under the Clinical Manager and participate in care planning, documentation, and family education to support recovery and quality of life. This role requires at least six months of SLP experience, an active license, CPR certification, and a valid driver’s license with transportation.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate and provide speech-language pathology services to patients in home or facility settings.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat speech, voice, language, hearing and swallowing disorders.
• Administer and interpret diagnostic tests and therapeutic applications.
• Document changes in patient condition and response to treatment.
