Job summary

CenterWell Home Health seeks a Speech-Language Pathologist to deliver evaluated therapy in patients’ homes or facilities and to participate in developing and reviewing care plans. You will assess and treat speech, voice, language, hearing and swallowing disorders and provide training on alternative communication methods.

The role requires CCC‑SLP/CCC‑A per ASHA, at least six months of relevant experience, and an unrestricted license plus CPR certification. Home health experience preferred.

Qualifications

• Must meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license; CPR certification on file.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize impairment.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in patient condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility.
• Prepare and submit written reports of evaluations, visits, summaries, care plans, and progress reports.
• Participate in care coordination activities and discharge planning.
