Job summary

Residential Home Health and Hospice in Dover, PA is seeking a Speech Language Pathologist (SLP) to provide skilled care in patients’ homes. This role requires travel within the assigned territory and offers a $5,000 first-year bonus for achieving key milestones.

You will perform comprehensive evaluations, develop individualized treatment plans, and educate patients and caregivers while documenting in the EMR.

Qualifications

• Active Speech Language Pathologist with SLP license to practice in the state of work.
• Minimum of 1 year of experience as an SLP.
• Valid driver's license required due to 20% travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered home health care in patients' homes.
• Perform comprehensive evaluations of speech, language, cognitive-communication and swallowing.
• Develop and implement individualized treatment plans.
• Educate patients and caregivers on exercises and safety strategies; document in EMR.
