Job summary

Residential Home Health and Hospice is seeking a Speech Language Pathologist (SLP) to provide skilled care to patients in their homes in the Dover, PA area. The role offers a $5,000 first year success bonus for achieving key milestones.

Qualifications

• Active SLP license in the state you work.
• One year of SLP clinical experience delivering patient care.
• Valid driver’s license and ability to drive ~20% of your workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations of speech, language, cognition, and swallowing.
• Develop individualized treatment plans to support safety, independence, and quality of life.
• Educate patients and caregivers on therapeutic exercises.
• Ensure thorough, compliant EMR documentation; think critically and solve problems.
• Work independently, but collaborate with the team and travel to patient homes in the territory.

• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
