Job summary

Residential Home Health and Hospice is seeking a Speech Language Pathologist (SLP) to provide skilled care to patients in their homes in the Lansdowne, PA area. This role offers a $5,000 first year success bonus and ongoing growth opportunities within a nationally recognized home health and hospice leader.

You will conduct evaluations, develop treatment plans, educate patients and caregivers, and document thoroughly in the EMR.

Qualifications

• Active Speech Language Pathologist with an SLP license in the work state.
• At least 1 year of experience as an SLP.
• Current driver’s license and ability to travel to patient locations about 20% of time.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting.
• Perform comprehensive evaluations of speech, language, cognitive-communication, and swallowing function.
• Develop individualized treatment plans to support safety, independence and quality of life.
• Educate and empower patients and caregivers on therapeutic exercises.
• Ensure compliant EMR documentation and collaborate with the interdisciplinary team.
• Travel to patient homes within assigned territory.

• Work independently, but never alone
• Travel to patient homes within assigned territory

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
