Job summary

Residential Home Health and Hospice in Michigan is seeking a Speech Language Pathologist (SLP) for a home health role serving Genesee County and surrounding areas. A $10,000 first-year success bonus is available to reward early achievements.

You will deliver skilled care, conduct evaluations, create individualized plans, educate patients and caregivers, and maintain compliant EMR documentation while traveling to patient homes within the territory.

Qualifications

• Active Speech Language Pathologist with SLP license in the state of work.
• Minimum of one year of clinical SLP experience delivering patient care.
• Valid driver’s license and willingness to drive to patient locations (~20% of time).

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life.
• Educate and empower patients and caregivers on therapeutic exercises.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
