Residential Home Health and Hospice is seeking a dedicated Physical Therapist to provide skilled, patient-centered care in patients' homes across Lansdowne, PA. This role offers autonomy, meaningful patient relationships, and growth opportunities, with a $10,000 first-year bonus and ongoing rewards for performance.

You'll evaluate mobility, design individualized therapy plans, educate patients and caregivers, and partner with an interdisciplinary team while traveling within your territory.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist.
• Valid driver’s license and ability to travel ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
