• Current driver's license and ability to spend ~20% of your day driving to/from patient locations

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Perform comprehensive evaluations of speech, language, cognitive-communication, and swallowing function with individualized treatment plans
• Educate patients and caregivers on therapeutic exercises and ensure compliant EMR documentation

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
