Job summary

Graham Healthcare Group's Residential Home Health and Hospice is seeking a Speech Language Pathologist to provide skilled care to patients in their homes in the Lancaster, PA area. The position includes a $5,000 first year bonus for achieving key milestones.

Qualifications

• Active Speech Language Pathologist with SLP license in the state you work.
• At least 1 year of experience as an SLP delivering patient care.
• Current driver’s license and ability to travel ~20% of the day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing treatment plans.
• Educate and empower patients and caregivers on therapeutic exercises.
• Ensure thorough, compliant EMR documentation.

What You’ll Do

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
