Job summary

Residential Home Health and Hospice is hiring a Speech Language Pathologist (SLP) to provide skilled home health care across the Lake Forest, Highland Park, Glenview, and surrounding areas. Expect to earn bonuses up to $10,500, including an $8,000 first-year success bonus and a $2,500 sign-on within 18 months, with ongoing growth opportunities.

In this field-based role, you’ll evaluate and treat patients in their homes, collaborate with an interdisciplinary team, and document thoroughly in EMR

Qualifications

• Active Speech Language Pathologist license in the state you work.
• Minimum one year of experience delivering patient care as an SLP.
• Valid driver’s license and ability to travel to patient locations (~20% of time).

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisplinary team.
• Conduct comprehensive evaluations of speech, language, cognition and swallowing; create individualized treatment plans.
• Educate patients and caregivers on exercises and strategies for safety and independence.
• Maintain thorough EMR documentation and meet regulatory requirements.
• Travel to patient homes within assigned territory and work independently when needed.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
